The opportunity
Silverchain Multidisciplinary Care Team are looking for an experienced Social Worker to join the our team on a permanent, full time basis. We may be open to a 4 day week at very minimum for this role. This role is based at Osborne Park (but open to other metro based Silverchain locations) between the hours 8.30am - 4.30pm (Or 8am - 4pm) The role is supporting all of WA and will be based metro region. There will be telehealth support for regional areas.
Key responsibilities
You will work closely with clients, their families and relevant others to take a holistic approach to the social and health needs of clients with an ultimate goal of reducing the number of unplanned admissions to hospitals by successfully integrating the care needs of each client.
About you
To be considered for this role, you must meet the following criteria:
- Tertiary qualification in Social Work and eligible membership of AASW (Australian Association of Social Workers)
- Previous experience working within palliative care
- The ability to work within a multidisciplinary team
- Experience in client assessments and care plans
- Excellent communication and interpersonal skills
- High computer literacy
