Occupational Therapist (Early Supported Discharge – Medical/Older Adults/Orthopaedics)

Contract: Permanent, Part Time 30 hours per week.

This is an opportunity for an experienced Occupational Therapist to make a real difference in patients’ lives by supporting their safe and successful transition from hospital to home.
The focus is on early supported discharge for medical, older adult, and orthopaedic patients, working in partnership with whānau to restore independence and wellbeing.
About the Role As part of a dynamic, multidisciplinary team, this role provides:
- Evidence-based rehabilitation to help patients safely return home sooner
- Functional assessments and tailored intervention planning
- Collaboration with nursing, medical, and allied health colleagues to ensure continuity of care
- Involvement in service improvements and innovative models of care delivery
About You The ideal candidate will bring:
- Experience in acute, medical, older adult, or orthopaedic rehabilitation
- Excellent clinical reasoning and communication skills
- A collaborative, patient-centred approach
- High levels of initiative and adaptability in a fast-paced environment
- Current Annual Practising Certificate (NZ OT Board)
- Full driver’s licence
