Job Title: Clinical Care Specialist

About The Role:
* You will be providing clinical in-home services, including care plan assessment, medication management, incontinence care, wound care, chronic disease management, and palliative care.
* You will supervise non-clinical team members in a community setting, process intakes, conduct client care assessments, and coordinate quality person-centred care according to client needs.
Requirements
* Qualified Registered Nurse or Enrolled Nurse with current AHPRA registration.
* First Aid & CPR certification, Australian National Police check, Professional Indemnity Insurance, and a valid Driver's Licence.

* Exceptional communication skills, able to work autonomously or within a team environment, and excellent time management and organisation skills.
