The Nurse Navigators at the Bowen Hospital and Collinsville Multi‐Purpose Health Service are seeking applications for a temporary Nurse Navigator to join their team.
Responsibilities
The purpose of this role is to demonstrate accountability at an advanced practice level for managing and coordinating ongoing care plans for relevant client groups within the geographical area, and for providing nursing leadership specialist advice, consultation and education to inter‐professional team members as relevant to the specific context of the role.
The Nurse Navigator function is a fundamental part of the nursing role, facilitating the patient journey in an increasingly complex health system by reducing fragmentation, mitigating barriers, educating, empowering and coordinating patient care. The role transcends across multiple specialties, supports nurse‐led service models and augments existing models of care within a facility or clinical area.
The Nurse Navigator reports to the Nurse Navigator (Team Lead) – Nurse Navigator Service, reporting through the Allied Health, Sub‐Acute and Ambulatory Service Division. Each Nurse Navigator will be responsible for patient‐centred care coordination of one of the identified key priority areas, and these positions will be flexible and adaptable to meet the needs of clients accessing the MHHS.
Utilization of current technology, including integrated electronic Medical Record (ieMR) and Telehealth as service enablers, is required.
