Position Overview
We are looking for a dynamic and experienced Support Coordination Team Leader to join our growing team. In this full-time role, you will lead and manage our Support Coordination team, ensuring the highest standards of service delivery to our NDIS participants. Reporting into the Client Services Manager you will have the opportunity to contribute to key strategic initiatives, mentor and develop your team, and shape the future of 360 Health Management. This role is based in our Head Office in Shepparton but will require travel to our other offices in Seymour, Bendigo and Swan Hill.

Key Responsibilities
1. Client Management: Working with participants to achieve their goals in line with their NDIS plan or those discussed individually with the participant. Maintain accurate and up to date case notes on participants progress, requests, and conversations.
2. Leadership & Team Management: Lead, mentor, and motivate the Support Coordination team to deliver best practice services. Conduct regular performance reviews and provide coaching to enhance team capabilities.
3. Stakeholder Management: Build and maintain strong relationships with internal and external stakeholders, including participants, carers, families, community organizations, and service providers.
4. Strategic Planning: Work closely with the Managing Director and leadership team to shape the strategic roadmap and drive key initiatives that deliver tangible value to the organization.
5. Service Delivery: Ensure that participant services are planned, executed, and monitored effectively. Develop Service Agreements and support schedules and liaise with specialists to ensure participant needs are met.
6. Innovation & Continuous Improvement: Foster a culture of innovation and continuous improvement within the team. Encourage new ideas and improvements in service delivery to enhance client outcomes.
7. Advocacy: Represent and advocate for 360 Health Management's brand and mission, amplifying our values and ensuring the team remains client-focused at all times.
8. KPI Achievement & Compliance: Ensure that each team member, including yourself, meets the 85% KPI target and adheres to compliance standards. Monitor team progress, provide necessary support, and address any challenges to reach set targets.
Key Skills & Qualifications
1. Leadership & Management: Proven experience in leading and managing teams, with the ability to motivate, inspire, and drive performance. Minimum 2 years of experience.
2. NDIS Knowledge: Strong understanding of the NDIS and its requirements, with experience in delivering support coordination services. Minimum 3-4 years of experience.
3. Excellent Communication Skills: Ability to communicate clearly and effectively in writing and verbally with a range of stakeholders.
4. Strategic Thinking: Ability to contribute to the strategic direction of the organization and implement effective strategies that align with the mission and goals.
5. Negotiation & Relationship Building: Skilled in negotiating with internal and external stakeholders and fostering strong professional relationships.
6. Problem Solving & Innovation: Strong problem-solving skills with the ability to innovate and continuously improve processes and service delivery.
