Responsibilities
* Meeting clients in their homes to understand what matters most to them.
* Designing care plans that reflect goals, preferences, and personality.
* Coordinating services, monitoring changes and managing budgets to keep everything on track.
* Supporting families and care teams with empathy and clear communication.
Qualifications
* Certificate IV in Ageing Support or Diploma in Nursing desirable.
* At least 2 years\' experience in home care coordination.
* Knowledge of Home Care Packages and the Support at Home Program.
* Strong organisational skills and proactive mindset.
