Employment Term: Permanent Full Time or Part Time
Location: Toowoomba Allied Health Team
Remuneration: $42.15 - $59.04 p/hr (dependent on experience) + 12% Super + Not for Profit Salary Packaging + Penalties & Allowances
5 weeks annual leave (Pro Rata for Part Time)

Responsibilities
- Provide person centred functional and environmental assessments and interventions to promote independence and quality of life. Working within the client’s home, offer specialised equipment and pressure care strategies.
- Manage a wide variety of client presentations across community aged care funding bodies including Home Care Packages, Commonwealth Home Support Program, Transitional Care Program and Palliative Care.
- Provide in home assessments and develop treatment plans to enable clients meet their goals. This includes interventions such as simple and complex equipment prescription, major and minor home modifications and client education.
- Work within a supportive statewide Occupational Therapy team of 80 plus OTs with decades of experience. Collaborate with a diverse multidisciplinary Allied Health team and Blue Care Neighborhoods including nurses, care workers and support staff.
About
- Tertiary Qualifications and current AHPRA registration
- Previous experience within an Aged Care or Acute setting
- Exceptional communication skills with the ability to build great relationships
- Effective time-management and proven clinical problem-solving skills
- Current Australian Drivers Licence and a road worthy vehicle with comprehensive insurance as required
