We are seeking a dedicated Senior Community Support Specialist to join our team. The ideal candidate will have experience working with diverse caseloads, collaborating with multidisciplinary teams, and providing specialist support in acute and outpatient settings.
About the Role
* Support adults, children, and families in acute and outpatient care.
* Manage dynamic, varied caseloads and collaborate with healthcare professionals to deliver high-quality patient outcomes.
Requirements
* AHPRA registered Social Worker (or eligible).
* Experience in hospital settings (adult and paediatrics) and a strong understanding of child protection and domestic violence cases.
* Ability to work independently and as part of a team, prioritizing tasks and managing time effectively.
* Strong communication and interpersonal skills, with the ability to build rapport with clients, colleagues, and other stakeholders.
