About the role
In September 2025 Western District Health Service (WDHS) and Casterton Memorial (CMH) reached an in principal agreement to work towards the merger of the two organisations. This agreement has resulted in the creation of a Merger Steering Committee to oversee the process and the development of initial key working groups.
The purpose of the merger is to see more high quality health services provided to the greatest possible number of community members as possible within the combined catchment areas. We intend to leverage the combined capabilities of the town health services to maximise the benefits and reach for our current and future clients.
This project covers the implementation of key governance, staffing, operational, organisational and health service change to align and integrate the two organisations. The workplan is expected to be implemented over a one-year period. It is possible the project may be extended to a second year pending confirmation of additional funding.
The Transition Project Manager will manage the project to implementation. The role will comprise of the following key elements:
* Provide open communication and consultation with staff and external stakeholders on changes and opportunities involved in the merger process.
* Create, manage and implement the WDHS/CMH merger work plan with support from staff and board.
* Ensure that WDHS-CMH merger work plan timelines, milestones and deliverables are archived.
* Work closely with Legal and Compliance teams to ensure adherence to regulatory and legal requirements.
* Report to and support the Merger Steering Committee.
* Develop clear and consistent communication channels.
* Oversee the execution of integration/separation tasks across all business functions.
* Coordinate Day 1 readiness and ensure a smooth transition.
* Support project evaluation and acquittal.
* Other tasks as directed and consistent with the merger project.

The successful candidate must have or obtain a National Criminal Police Check. Depending on the role, a Working with Children Check Clearance, Disability Worker Exclusion Scheme check, and/or NDIS Worker Screening Check may also be required. All employees must comply with WDHS vaccination requirements. WDHS is a smoke-free workplace.
