Abilifly Support Services is a registered National Disability Insurance Scheme (NDIS) provider with an office in Cairns. We specialise in providing in-home support and community support/access to people with a disability, aged, children and young adults. We are seeking physically fit, active, energetic, enthusiastic, and professional individuals with solid experience in the community and/or disability sector to join our team in Cairns on a full-time permanent basis.
Reporting to the Service Delivery Officer/Manager, you will support participants to reach their individual goals through meaningful and fun activities in their home and out in the community.
As a Support Worker you will:
1. Assist people to perform important daily activities.
2. Facilitate daily personal hygiene and dressing tasks.
3. Perform home-based tasks such as food preparation and housework.
4. Undertake tasks outside the home such as shopping.
5. Ensure a varied program of social activities.
6. Provide emotional support and friendship.
7. Support clients to participate in age-appropriate activities and tasks including employment, in-home support and community access.
8. Support clients to access activities and outings in the community.
9. Provide direct care assistance to each client in accordance with their service plan and as directed by the Manager.
10. Work in accordance with the medication administration policy, work health and safety procedures, and client rights policies including privacy and confidentiality.
11. Monitor the safety and well-being of the clients and report any concerns to the Manager.
12. Provide 1:1 or small group training and support in response to individual client needs.
13. Ensure completion of appropriate documentation.
14. Record relevant information on support provided in client notes and communication book as required.
15. Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
16. Work with clients, family/carers, relevant case managers and other professionals to support the client's individual goals.
17. Assist in developing, implementing and evaluating training and behaviour programs to attain identified individual goals.
18. Ensure that all programming undertaken is appropriately maintained, documented and reviewed.
19. Assume the role of key worker for designated clients, including the preparation of a monthly report on client progress.
20. Some travel may be required to clients who are outside of the Cairns region.
Essential Criteria:
1. Physically fit and active.
2. The ability to develop a positive relationship with the participant.
3. Excellent communication skills.
4. The ability to work with children with disabilities.
5. Able to work in a fast-paced environment.
6. Ability to acquire NDIS Worker Screening Check.
7. Ability to acquire a Working With Children's Check.
8. Covid-19 Vaccination Certificate.
9. First Aid Certificate is desirable.
10. Cert III in Individual Support or Cert IV in Disability (or equivalent) is desirable.
