Referral enables care to be implemented in the patient’s home environment, avoiding unnecessary transfer to hospital, and leading to improved patient outcomes.The senior social worker provides clinical governance, leadership and expertise within social work services including the provision of safe, timely, compassionate and high quality person-centered care. The position will be responsible for providing care to keep consumers well in the community by providing care to facilitate early, safe discharges from hospital and reducing unnecessary admissions to hospital.What You'll be DoingThis position is responsible for providing specialist Social Work service to keep consumers and their carers well in the community, reducing hospital admissions and facilitating safe, early discharges from hospital. The position will ensure the provision of optimal health outcomes consistent with SWSLHD policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct. View our application guide for information on how to respond to criteria and improve your application .A Social Work degree which provides eligibility for membership of the Australian Association of Social Workers (AASW).Extensive post graduate experience working as a Social Worker with community outpatient and/or people with chronic diseases, including a demonstrated understanding of the health setting, complex health conditions and the impact on wellbeing.Advanced clinical knowledge and postgraduate experience and skills in psychosocial assessments and interventions to minimise psychosocial risk for patients, carers and families, including identifying and responding to Older Persons abuse and facilitation of applications to NSW NCAT.Demonstrated high level organisational and time management skills, along with excellent written and verbal communication skills.Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment.
