Support Coordinator Role
The role of Support Coordinator is to oversee and coordinate the operational running and program delivery at Horizon House accommodation locations, including associated transition placements.
* Operational coordination involves managing day-to-day activities and leading teams to achieve program goals.
* Recruitment of caregivers includes facilitating inductions and providing support for new staff members.
* Facilitating regular team meetings includes case discussions and service coordination. Providing line management of team members includes regular supervision, performance development, and individual de-briefing as required.
* Supporting a trauma-informed approach involves embedding principles with all caregivers in this area.
* Collaborating with the Practice Lead to prepare and assess new referrals for presentation at Assessment Panel meetings.
* Case management involves developing and reviewing client plans with each program participant, assisting and supporting transitions and case management of participants moving in, out, and between different service tiers.
* Developing strong networks within the local community and actively promoting services.
* Supporting the State Manager to monitor and maintain resources; manage the day-to-day running, general maintenance, reporting requirements, and capital expenditure of the accommodation services within the designated area.
Required Skills and Qualifications
You will have a well-developed understanding of trauma-informed care including theoretical perspectives such as loss and grief, attachment, resilience, and connectedness. Child & adolescent development and the capacity to apply a range of practice responses are also essential.
Must have a sound understanding of the challenges involved in service delivery to young people in the homelessness sector and experience in program coordination and leadership of a team to support program delivery.

Successful applicants will hold a tertiary qualification in Youth Work, Social Work, or Psychology and a minimum of 5 years' experience working with young people in a residential care setting, current & valid first aid & CPR, and full un-restricted Driver's License.
