Dementia Care Consultant

Pay: $59.43 to $60.70 per hour
Responsibilities
* Deliver direct in‐home or telehealth support to people living with Dementia and their families, providing information and tools specific to individual circumstances.
* Assist clients in accessing BlueCare support services and programmes to maintain health and wellbeing.
* Offer carer support and techniques to cope with individual behavioural concerns.
* Prescribe meaningful activities and equipment from a comfort interventions library to engage people living with Dementia.
* Perform care planning and assessment following review schedules and changes in clients' needs.
* Coordinate linkages and referrals to external stakeholders and government departments for specialised dementia care where required.
* Advocate for person‐centred care, providing high‐quality advice that supports clients' self‐management and independent living.
Qualifications
* Degree‐level qualifications in Nursing, Health, Community Service or a relevant discipline, or extensive relevant work experience.
* Certifications or qualifications in dementia care.
* Current AHPRA registration.
* Extensive specialist knowledge and experience in dementia assessment, behaviour management planning, care planning, implementation and evaluation, and mentoring and supervision of allied health or human services staff.
* Expert knowledge of services, tools, techniques and resources to assist clients and staff.
* Proven leadership capability within a multidisciplinary team.
* Comprehensive understanding of client‐centred, community‐based care service delivery.
* Ability to liaise with the clinical and care team and provide coordinated care.
* Experience and knowledge of community funding streams and programmes.
* Comprehensive knowledge of legislative, regulatory, professional standards and guidelines relevant to the aged care community services environment, including quality systems, accreditation, WHS and continuous quality improvement.
* Effective interpersonal and written communication skills, including intermediate computer skills (e.g., Microsoft Office Suite).
