Support Coordinator – Care Connect (Part-time)
Join Social Futures' Universal Aftercare program, Care Connect, in Singleton to provide therapeutic supports and short-term psychological interventions for people experiencing a suicidal crisis or who have attempted suicide.
Location: Singleton (part-time, flexible hours and days).
As a Support Coordinator you will:
Provide therapeutic supports and short-term psychological interventions, connecting participants to services and supports.
Expand your case management and care coordination skills.
Work as part of a multidisciplinary team to deliver evidence-based, high-quality services.
Build partnerships with other community services and expand your network.

Qualifications:
Diploma or Certificate IV level qualification in Mental Health or Community Services.
At least two years' experience in a suicide prevention/aftercare or mental health setting.
Previous experience providing case management to vulnerable groups.
Demonstrated ability to maintain professional boundaries while engaging in person-centred work.
