About the Role
The role of an Aged Care On-site Pharmacist is to provide collaborative and integrated care for residents in residential aged care facilities. The successful candidate will work closely with healthcare professionals, including GPs, nurses, facility staff, and other pharmacists.
The pharmacist will develop a working relationship with the operations manager and integrate within the team, both internally and externally. They will provide expertise in medication management, focusing on person-centred care and improving health outcomes for residents while reducing medication-related harm and optimising medicines use.
The key responsibilities of this role include:
* Providing continuity in medication management and solving medication-related issues
* Assisting with medication management and communication during transitions of care
* Undertaking quality use of medicines activities across the facility
* Being a central contact for medication management enquiries
To Be Successful You Will Have
* A valid Australian Health Practitioner Regulatory Agency (AHPRA) registration as a pharmacist
* Credentialing having completed an accredited training program for Aged Care On-site Pharmacists
