About the Role
We are seeking an experienced Community Health Practitioner to join our team. As a key member of our healthcare team, you will be responsible for delivering high-quality community nursing and health services to individuals and communities.
Key Responsibilities
* Provide community nursing and health services to individuals and communities.
* Promote and maintain collaborative relationships with Department of Education and other stakeholders.
* Develop and implement effective community health programs and services.
* Collaborate with multidisciplinary teams to achieve health outcomes.
Requirements
To be successful in this role, you will need:
* Eligibility for registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
* Recent postgraduate nursing experience and demonstrated advanced nursing knowledge and skills.
* Demonstrated well-developed interpersonal skills and ability to communicate effectively with diverse populations.
* Understanding of primary health care principles and their application to community health service delivery.
