Senior Community Nurse
Job Summary

Key Responsibilities

* Implementation and Evaluation: Deliver high-quality community nursing care, working closely with patients, carers, and the healthcare team to ensure seamless care coordination.
* Communication and Collaboration: Develop and maintain effective relationships with patients, carers, and healthcare professionals, ensuring open communication and collaboration to achieve quality patient outcomes.
Requirements
* Registration: Current registration as a Registered Nurse (Division 1) with the Australian Health Practitioner Regulation Agency (AHPRA).
* Experience: Proven experience working in community nursing, with a strong understanding of community health principles and practices.
* Clinical Skills: Demonstrated ability to assess, plan, implement, and evaluate community nursing care, utilizing clinical skills and knowledge to deliver high-quality patient outcomes.
* Interpersonal Skills: Excellent communication and interpersonal skills, with the ability to build effective relationships with patients, carers, and healthcare professionals.
