Key Responsibilities
* Deliver comprehensive psychosocial interventions to patients and their families, addressing their diverse needs and concerns.
* Undertake in-depth psychosocial assessments to inform treatment decisions and facilitate family meetings that promote collaboration and understanding.
* Process and manage referrals through the Electronic Journey Board, ensuring timely and effective support for patients and their families.
* Contribute to patient-centred care in both inpatient and rehabilitation settings, prioritizing individualised support and advocacy.
Requirements
* A bachelor's degree in social work is required, with eligibility for membership of the Australian Association of Social Workers or equivalent qualification as deemed by the employer.
* Candidates must hold Australian Citizenship, permanent residency, or New Zealand citizenship with a valid passport.
