About the Role
As a Disability Services Planner, you will be responsible for managing a portfolio of client records, ensuring high-quality and timely support is delivered. Your primary duties will include:
* Developing and reviewing comprehensive care plans based on individual needs and goals.
* Managing and prioritizing a demanding workload to meet key service delivery targets.
* Maintaining accurate and detailed client records using IT and data management systems.
* Engaging with a range of internal and external stakeholders to facilitate positive outcomes.
* Utilising your conflict resolution skills to manage difficult conversations and complex situations with tact and empathy.
* Conducting data analysis to inform decisions and contribute to service improvement.
