Position: Transition Support Officer

Salary: Level 3, $81,448 - $87,376 per annum (pro-rata) (DOE (SSO) CA 2024)

This is a permanent full‑time position commencing in Term 4, 2025.

Role purpose

The successful applicant will support the coordination of the student transition program, including organising parent information sessions and assisting in developing and introducing systems and intervention strategies that provide a responsive and effective support service for students, families and staff.

The role includes providing support and advice to staff on strategies for supporting students, facilitating group work processes, and developing links between students, families, the school, feeder schools and other agencies in the community.

The role also involves establishing a network of partnerships with government and non‑government agencies to ensure effective transition support for students with disability.

Key responsibilities

- Assist in coordinating the student transition program and related activities.

- Organise parent information sessions and support the development of system and intervention strategies.

- Provide support and advice to staff on strategies for supporting students; facilitate group work processes.

- Develop and maintain links between students, families, the school, feeder schools and other appropriate agencies.

- Establish partnerships with government and non‑government agencies to support transition for students with disability.

- Undertake assessments on students; liaise with parents/guardians, students, staff and other relevant parties; maintain student-specific case notes, briefing notes and reports as required.

- Identify student aspirations and interests to develop activities and projects for transitioning students; undertake research on current issues for young people; participate in projects and provide written reports as required.

- Oversee enrolment procedures, including operation of databases and information management systems and provision of enrolment information to students and families.

Qualifications and selection criteria

- Experience in coordinating student transition programs and working with students with disabilities.

- Ability to liaise with parents/guardians, students, staff and other stakeholders; maintain accurate case notes and reports.

- Strong written and oral communication skills; ability to establish and maintain collaborative working relationships.

- Effective planning and organisational skills; ability to manage competing priorities within timelines.

- Knowledge of youth services and local organisations; ability to conduct research and provide recommendations.

- Understanding of enrolment procedures and data management.
