About the role We are seeking Health Care Workers to join our innovative and committed medical team. The successful applicants will provide high standard services that support and assist patients and residents to achieve maximum independence by meeting daily living and personal requirements within their plan of care, with consideration of individual preferences. The role will assist patients and residents to maintain their independence wherever possible and to provide daily care using a person-centred approach in accordance with the resident's individual care plans. Hours of work will include shift work, weekends and public holidays.
