The Disability Navigator role is a crucial part of our healthcare team, requiring exceptional care coordination and case management skills to ensure successful discharge from hospital for individuals with disabilities.
This position involves providing specialist intellectual disability health team outreach services, supporting NDIS eligible individuals in hospital, and expediting access to NDIS services to alleviate pressure on FWLHD services.
Key Responsibilities:
* Providing care coordination, case management, and navigation for people with disabilities in hospital to ensure appropriate supports for discharge.
* Supporting NDIS eligible individuals who are in hospital, at risk of hospitalization, or where support will expedite access to NDIS services.
Requirements:
* Tertiary qualification and 3 years clinical experience as an Allied Health Clinician, preferably in Social Work or a relevant Allied Health profession.
* Demonstrated advanced clinical reasoning, experience, and practice skills relevant to assessment and treatment of people with disabilities.
* Strong understanding of navigating the NDIS and alternative disability-related supports.
* Ability to coordinate and communicate complex care plans within a healthcare context, as part of a multi-disciplinary team.
* Effective negotiation, advocacy, collaboration, and communication skills with a wide range of stakeholders.
