Salary Range: $76,515 - $80,000 | depending on skills level and work experience + superannuation.

About the Role
Our client delivers person-centered support that helps individuals live independently at home and stay connected to their community. As their Community Worker, you will empower clients – including seniors, people with disabilities, and those with complex needs – to achieve their goals and participate fully in community life.
Key Responsibility and Duties
Client assessment and planning
- Conduct holistic assessments of each client's physical, emotional, and social needs, identifying strengths and barriers to independence.
- Develop, implement, and regularly review individualized support, training, and educational programs that align with client goals and NDIS/aged-care plans.
Client support and progress monitoring
- Interview the clients to understand the nature and extent of their difficulties, then provide direct support or skill-building to resolve social, emotional, and financial challenges.
- Monitor client progress through regular check-ins and documented observations, reporting outcomes to the care team, family, and funding bodies.
Community engagement and referrals
- Assess local community needs and available resources for health, housing, employment, training, and social activities, then connect clients to relevant services.
- Liaise with community groups, welfare agencies, government bodies, and businesses to promote awareness of support services and advocate for client access.
Program development and administration
- Prepare detailed submissions for funding, resources, and program extensions, along with progress reports for government and referring agencies.
- Supervise and support clients on probation, parole, or in adult service units, group housing, or government institutions – ensuring compliance and rehabilitation goals.
Requirements
- 1 year of full-time work experience within community services, social work, or in related positions.
- Relevant qualifications are required.
- Strong assessment, communication, and advocacy skills.
- Ability to work with vulnerable populations and maintain professional boundaries.
- Current driver's license and willingness to travel across Brisbane.
