Posted: 16 June
The role
  • A rewarding opportunity to make a real impact on heart health.
  • Flexible hybrid working arrangements from any of our Capital City offices.
  • Part Time (0.6) contract role – ending 18 Dec 2026.

About Us – Have a Heart for Change!

At Heart Foundation, we believe everyone deserves a healthy heart. Heart disease is one of the nation's leading causes of death, and since 1959 we've led the fight through research, support, and prevention. Be part of our mission to make heart health achievable by everyone in Australia by 2050.

How You Can Make a Difference

Due to internal secondment, we're looking for a part time Community Grants Coordinator to support the Active Australia Innovation Challenge, a national grants program that helps communities get more active.

This role sits across an important transition point in the program. You'll support current grant recipients to successfully wrap up their projects, ensuring strong outcomes, clear reporting, and celebrating the impact they've made in their communities. At the same time, you'll help prepare for the next round, setting up incoming grant recipients for success and ensuring the program continues to run smoothly.

It's a great fit if you enjoy coordinating projects already in motion, working closely with community partners, and delivering meaningful outcomes, while also keeping an eye on what's coming next.

You will have the opportunity to work flexibly, blending your time between home and any of our capital city offices.

What You'll Be Doing

  • Coordinate the final stages of current community grant programs, ensuring milestones, budgets and reporting requirements are met
  • Provide hands‐on support to grant recipients as they complete and evaluate their projects
  • Prepare for the next grant cycle, supporting onboarding and readiness for incoming recipients
  • Manage grant administration and reporting through SmartSimple
  • Partner with internal teams (Finance, Marketing, Digital) to support program delivery
  • Capture and share program impact through case studies, including working with videographers and photographers
  • Contribute to program evaluation and continuous improvement

What You'll Bring

  • Experience coordinating projects or programs, ideally in a grants, community or health promotion environment
  • Strong organisational skills with the ability to manage competing priorities across different program phases
  • Experience supporting or working with grant recipients or community partners
  • Confidence working with budgets, reporting and administrative systems (experience with grant platforms like SmartSimple is a bonus)
  • Strong communication skills and ability to build effective stakeholder relationships
  • A practical, delivery-focused mindset. You enjoy getting things over the line while setting up what comes next
