About this role:
We are seeking two Occupational Therapists to join our team in a rewarding position that supports older adults to regain their independence after a hospital stay.
This role is part of our short-term Transition Care Program, which helps older people restore their functional independence and either return home or transition to residential care.
The successful candidates will work across our Transition Care Program facilities and in the community, supporting clients with various needs.
* Conduct client assessments to determine capabilities, goals, and support needs
* Deliver tailored, goal-oriented therapy to support independence
* Prescribe aids and equipment as required
* Provide guidance to Therapy Assistants
* Communicate effectively with clients, families, and multidisciplinary teams
* Collaborate across the organization to ensure high-quality care delivery
* Contribute to quality improvement and best practice initiatives
Key Requirements:
* Current registration as an Occupational Therapist with AHPRA (essential)
* Valid Australian driver's licence and willingness to travel between sites (essential)
* Previous experience in aged care, community health, or rehabilitation settings (desirable)
* Demonstrated ability to conduct functional assessments and develop person-centred therapy plans
* Experience working with clients with dementia, chronic illness, or mobility limitations (highly regarded)
* Knowledge of aged care quality standards, funding models, and clinical documentation
* Ability to work independently and as part of a multidisciplinary team
* Effective time management and organisational skills to manage a varied caseload
