About the Role As Manager (Care Planning & Practice), you'll lead a team of Support Workers caring for our clients across Disability and Child Safety clients. Your role focuses on delivering exceptional care, ensuring compliance with regulatory frameworks, and fostering a positive team culture. You'll work closely with health professionals and the Senior Manager to implement and evaluate care practices that meet our high standards. and Disability Support clients, ensuring clinical compliance, and driving a positive and supportive team culture. Responsibilities include but are not limited to: Provide high-quality, professional care management advice and assistance to the DCR Principals and the Executive Manager (Care Policy and Practice). Oversee care management for our clients who may be adults with intellectual disabilities, some of whom may have a dual diagnosis, challenging behaviours, chronic illness or within our Child Safety portfolio and subject to Child Protection Orders arrangements and other impacting circumstances. Produce effective and clear care planning and practice support frameworks, including the individualised training and implementation of care plans. Produce clear and factual reporting within the required timeframes and in suit with relevant regulations. Ensure that behaviour recording is completed and submitted, ensuring that support staff have clear and defined knowledge of client behaviours. Develop in association with other health professionals, care treatment regimens for clients with medical, mental health and/or complex developmental and behavioural issues Develop in association with other health professionals, care treatment regimens for clients with medical, mental health and/or complex developmental and behavioural issues. Be responsible for the Conduct regular supervision and training of DCR care staff on-site and prepare official reports and documentation. Complete Client Needs Assessments monthly and as required. Participate, on regular rotation, in an after-hours' contact roster to provide professional advice to DCR care staff in managing critical incidents and matters affecting clients care and support requirements. (this includes critical incident reporting for all emergency events) The Successful Candidate Will Have: Tertiary qualifications in Psychology, Nursing, Social Work, Human Services, Special Education, or similar. Minimum 2 years' experience in care/case management. Knowledge of legislation and practice in at least one of the following: Disability Care, Forensic Disability, Child Protection, Aged Care, or Health Care. Queensland Driver License. Must hold or be willing to obtain: First Aid & CPR, Blue Card, NDIS Worker Screening Check, and LCS2 Background Check In return, we offer: Join a compassionate team in our Care Operations division.
