**About the Role**
As a Quality Improvement Partner, you will support the implementation of strategies and collateral for the agreed repeatable models of care and clinical service improvements. You will provide clinical education, continuous improvement, remediation coaching and mentoring. You assist with the roll out of clinical education programs tailored to clinical and professional leadership development for nurses and care home teams.

**Duties**
- based activities
- Monitor and report upon variance to targets (performance), making recommendations for clinical service improvementDevelop new procedures and work instructions supporting new models of care, improvements and clinical practice
- Develop new procedures and work instructions supporting new models of care, improvements and clinical practice
- Review clinical improvement activities, incidents in Riskman and assist GM and CCMs to improve the quality of the PCIs/CAPs and actions.
- Review and improve change management and implementation strategies, and reporting changes/improvements to Head of Clinical Governance and Development
- Develop and improve clinical training programs and modules as required for the organisation, including any current Nurse Development Programs
- Provide education for staff at a home level to support improved clinical care outcomes
**Skills & Experience**
- Bachelor of Nursing with current registration with AHPRA
- Qualifications in training and assessment, or equivalent experience
- Significant experience in managing a Care Home
- Experience in developing models of care / change management strategies
- Experience in implementing defined change processes
- Highly developed communication and problem-solving skills
- Experience in developing policy, procedures and work instructions
- Excellent project management, interpersonal and presentation skills
- Experience in developing and delivering education strategies supporting change processes
