Join our Albany Outreach Team as an Outreach Support Worker, supporting individuals living with neurological conditions such as MS, Parkinson's, Huntington's, MND, ABI, and Stroke. You'll promote and foster community engagement in a safe, social environment where clients can interact and build connections.
What You'll Be Doing
Lead and support outreach activities that promote social connection and engagement
Assist the Kitchen Supervisor in preparing and serving meals, ensuring a welcoming dining experience
Deliver exceptional service with a strong focus on enhancing the client experience
Follow individual care plans to meet each client's unique needs and preferences
Provide mobility assistance to support safe and comfortable movement
Stay attuned to emotional and physical needs, ensuring care is person-centred and responsive
About You
You're a compassionate, reliable, and proactive individual who's passionate about making a difference in the lives of others. You thrive in a team environment and are committed to delivering high-quality, person-centred care.
What You Bring
A genuine passion for supporting people with disability or the frail aged
Strong communication skills—both written and verbal
Confidence using systems and technology in daily work
NDIS Worker Screening Check (or willingness to obtain)
Desirable
Certificate III in Individual Support, Disability, Aged Care, or a related field
Current First Aid certification
Experience working in centre-based settings or with people with disability
