**About the role**
As an Case Manager, you will support children and young people to achieve their case plan goals, including supporting them to be restored to their family, live independently, move to a permanent home or step down to less intensive placement types.
To be successful in this role, you will have experience working with Children & Young People in out of home care and an understanding of trauma and its effects on children and young people. This is a hybrid working role, and regular travel across Western and South West Sydney in suburbs such as Fairfield, Campbelltown and Blacktown is a requirement of this role.
**Key Responsibilities**
- Develop and implement high quality therapeutic case plans for Children and Young People in LWB's care
- Develop leaving care plans for each young person over 16 to support their transition to independent living
- Attend shifts within the intensive therapeutic care homes, and work closely with the team
- Drive the implementation of Leaving care Plans / P4I Plans / Cultural Support Plans and achievement of the case plan goals
- Actively engage children, young people, their families and link them in with the local community services
- Participate in an on-call roster as required
- Ensure compliance with all internal policy and procedure, statutory and contractual requirements in relation to management of service delivery
**Skills & Experience**
- Relevant bachelor's degree, or a relevant diploma and be working towards a degree. (_A qualification is desirable but not essential for Aboriginal and/or Torres Strait Islander applicants with relevant experience and a willingness to undertake further training)_
- Previous case management experience within an Intensive Therapeutic Care or Out Of Home Care (OOHC) setting
- Experience supporting Children & Young People with disability, who have experienced backgrounds of trauma, abuse and/or neglect.
- Understanding of current casework practices for children and young people
- Current drivers' licence
