Overview
Registered Nurse – Care Coordinator position at Churches of Christ Queensland, Bundaberg, at Gracehaven (115-bed Residential Aged Care home).
Responsibilities
* Coordinating and liaising with external providers
* Overseeing and coordinating the completion of baseline and focus assessments
* Developing and reviewing care plans
* Coordinating the collection of accurate clinical and care data for timely submission and reporting
* Provide clinical leadership and support our RNs in developing clinical leadership skills
* Assist the Clinical Manager with follow up and provide oversight in quality clinical care
About You
Essential:
* Passionate about Aged Care and providing safe supportive care to the residents
* Has current AHPRA registration as a Registered Nurse (RN)
* Minimum 3 years clinical experience in the Aged Care Sector
* Current driver licence
* Currently holds, or willing to obtain a current Police Check and Blue Card
* Hold valid Australian Work Rights – Unfortunately we are not currently able to sponsor
Desirable
* Post-graduate qualifications in Geriatrics, Dementia or other Aged Care specialisation
* Currently hold, or willing to obtain a current First Aid and CPR Certificate
* Currently hold, or willing to obtain a current NDIS check
