A mental health professional is sought to join the Continuing Care Team within Manning Mental Health Service.
Job Description
The successful candidate will provide high-quality assessment, care coordination, intervention and follow-up for adults experiencing mental health concerns. They will work collaboratively with consumers, their families/carers, GPs, Community Managed Organisations (CMOs) and other service providers to ensure safe, effective, recovery-focused care.
Key Responsibilities
* Provide assessment and care coordination services to consumers and their families/carers
* Work collaboratively with multidisciplinary teams to achieve desired outcomes
* Develop and implement individualized care plans that promote consumer well-being and independence
* Communicate effectively with consumers, families/carers, and other stakeholders to ensure optimal care delivery
* Contribute to the development of policies and procedures that promote best practice in mental health care
Requirements
The successful candidate will hold a recognized and accredited tertiary qualification and registration (or eligibility for) with the relevant board via AHPRA, or eligibility for membership of the relevant association, in one of the following disciplines: social work, psychology, occupational therapy, dietitian or nursing.
