Overview
Belong at Home is seeking enthusiastic, motivated and caring people to join our South West team as a Community Nurse/Clinical Care Partner (RN).
This is a permanent part-time role with the potential to transition into full-time, offering flexibility and purpose in a community-focused setting.
Responsibilities
- Collaboratively support BaH clients approved for clinical service types under Support at Home.
- Support admissions of SaH clients into Belong at Home.
- Tasks associated with Clinical Care Partner defined within Support at Home program.
- Undertake clinical assessments of home care clients.
- Providing excellent care and service to clients in line with their personalised care plans.
- Monitoring chronic diseases and palliative conditions.
- Wound care, continence care, palliative care, medication management, restorative care and management of chronic diseases.
- A team player you will work closely with our care partners to ensure quality service delivery.
- Achieving the optimum care for clients by being responsive to their changing needs.
- Liaising with and referring to other organisations as required to provide for the clients needs.
- Assist the clinical leads with other duties as required.
Qualifications & Requirements
- Current AHPRA registration.
- Availability to work (M-F) – (ad hoc weekends where required). Minimum 3 days a week.
- Aged Care experience is an advantage.
- Excellent verbal and written communication skills.
- A genuine enthusiasm, care and compassion in your commitment to working with the elderly.
- A National Police Clearance is required.
