The role

About The Role

As a Nurse Assessor, you will deliver high‐quality, person‐centred nursing care to clients living with a range of neurological conditions including multiple sclerosis, Parkinson's, Huntington's, motor neuron disease, acquired brain injury and stroke. Your day will involve completing initial nursing assessments, developing care plans and collecting clinical information to ensure safe nursing services. You will be the first point of contact for clients and their carers, providing compassionate, person‐centred guidance right from the first interaction.

Key Responsibilities

  • Partner with GPs, pharmacists, and specialists to create comprehensive care plans that address all of the client's needs.
  • Keep medication records up‐to‐date and work closely with healthcare teams to provide ongoing care adjustments as necessary.
  • Provide physical and emotional support to clients and families, ensuring they feel heard, valued, and understood.
  • Work alongside an expert team of healthcare professionals to ensure the best possible outcomes for individuals living with neurological conditions.
  • Guide new clients through the onboarding process, ensuring a smooth transition and personalised support from day one.
  • Conduct in‐depth initial assessments, develop tailored care plans, and ensure clients receive the highest standard of care.
  • Travel throughout the community to support clients (pool vehicles available).

What's on Offer

We have a permanent part‐time position (60 hours per fortnight) for either an Enrolled Nurse or Registered Nurse at various stages of their careers. These roles will be based at either the Butler or Wilson Service Centre.

For Enrolled Nurses, we are looking for individuals with 3–4 years of experience, ideally with exposure to the disability and/or neurological sector.

For Registered Nurses, we are open to graduate RNs as well as those with more developed clinical experience. Any experience in the disability and/or neurological sector will be highly valued.
