**SERVICE DESCRIPTION**
Community Support Services support people in regional communities across Eastern Australia to live better lives
through social and community participation, coaching and capacity building, service coordination and transport
services. We support parents, carers, families, children and adults with disability, aged people, transport
disadvantaged, and small regional communities.
The Coordinator, Community Supports is responsible for providing leadership and management of a support team
delivering community-based social supports. These may occur in a group or individual setting. This includes leading a
team to design and deliver new and distinctive programs and services that support the acquisition of new skills and
experiences for our aged customers and people with a disability. The Coordinator, Community Supports is
responsible for the development, management and continuous quality improvement of community access
participant supports, maintenance of assigned programs, implementation of support plans, and resource
management including the rostering of frontline staff.
**RELATIONSHIPS**
- Reporting to either Manager, CORE Services or Head of Social & Community Support (depending on
location).
- Working closely with managers, rostering, coordinators, staff, customers, and their families.
- Building and maintaining relationships with all external stakeholders.
**MANDATORY QUALIFICATIONS AND EXPERIENCE**
- Certificate IV in Individual Support and/or Leadership, or equivalent and relevant experience.
- Current unrestricted Australian Drivers Licence and willingness to undertake travel in accordance with
responsibilities of the role.
- Current First Aid and CPR Certificates
- Completion of NDIS Worker Orientation Module 'Quality, Safety and You'
- Availability of a fully registered and comprehensively insured vehicle.
**Desirable**
- Diploma in Leadership & Management, or willing to undertake.
- Experience in service management including program design in aged services, disability services, mental
health service and/or children's services.
