About the Team
CHQ at Home provides a range of in‐the‐home health care services for children and young people living in the broader Brisbane metropolitan area. It includes Hospital in the Home (HITH), Post Acute Care (PAC), Hospital Avoidance (HA) and Palliative Care Programs.
HITH provides care in the home environment as an alternative to hospital inpatient stay for infants and children whose condition is secure or predictable. Patients can be transferred from an inpatient ward, shortening their length of stay in hospital, or admitted directly to HITH from an outpatient clinic or emergency department, avoiding an inpatient hospital stay altogether.

* Contribute to a workforce culture of patient safety, where patient safety is viewed as a top priority by staff, patients, families and the public, and staff are personally accountable for identifying, reporting, and escalating clinical risks and incidents, learning from experiences and proactively engaged in identifying and progressing improvement opportunities aimed at improving patient safety and the quality of care.
