Join Better Health Network and play a vital role in supporting Aboriginal and Torres Strait Islander peoples living with chronic health conditions. This funded program is focused on improving health outcomes through culturally safe care coordination, advocacy, and community connection.

We are seeking a compassionate and experienced Health Care Coordinator to work collaboratively with clients, families, community services, and multidisciplinary teams to deliver holistic, person-centred care.

About The Role

As the Care Coordinator - Indigenous Access, you will support Aboriginal and Torres Strait Islander clients to better manage chronic conditions and access the services they need to thrive. You will build trusted relationships within the local ATSI community and work closely with internal and external stakeholders to strengthen pathways to care and improve wellbeing outcomes.

Key Responsibilities

* Coordinate comprehensive care plans and case management support for clients with complex needs
* Conduct client assessments and support goal setting and self-management
* Connect clients with specialist supports including housing, mental health, AOD, legal and social services
* Build and maintain strong partnerships with ATSI organisations, networks, and community groups
* Participate in local Aboriginal health forums and community engagement activities
* Maintain accurate documentation, reports, and client records
* Identify service gaps and contribute to continuous improvement initiatives

About You

You are a relationship-focused and culturally aware professional who values collaboration, advocacy, and community-led care. You bring strong clinical knowledge alongside the ability to engage respectfully and effectively with Aboriginal and Torres Strait Islander communities.

* Registered Nurse Division 1 with current AHPRA registration
* Minimum 3 years post-graduate experience, ideally within community health or care coordination
* Experience working with Aboriginal and Torres Strait Islander peoples and clients with complex needs
* Strong understanding of the social and historical factors impacting ATSI health and wellbeing
* Demonstrated ability to build effective partnerships and engage stakeholders
* Excellent communication, case management, and report writing skills
* Postgraduate qualifications or further training in relevant health or community services fields
* Experience in chronic disease management and integrated care programs
* Leadership, mentoring, or project coordination experience
