Clinical Care Coordinator Job Opportunity
We are seeking a qualified Clinical Care Coordinator to work closely with our clinical team in an aged care residence based in Mandurah.
The ideal candidate will provide day-to-day clinical leadership, foster a culture of excellence and oversee the clinical care of residents ensuring a high standard of care is maintained.
* Provide clinical guidance and mentorship to a team of registered nurses, enrolled nurses and personal care assistants
* Be a role model for clinical leadership and professionalism
Key Responsibilities:
* A minimum of 2+ years experience in a clinical care coordination or similar role in the aged care or acute sector
* Registered nurse qualification with current registration
* Up-to-date knowledge of clinical assessment and staff supervision skills
* Excellent communication and negotiation skills
* A working knowledge of funding and accreditation processes
* Knowledge of care planning principles
