We are seeking experienced and compassionate General Practitioners to join our dynamic team. This role is ideal for GPs passionate about community health and home-based care. The GP will be responsible for conducting medical assessments and management plans directly in patients' homes, improving their quality of life through proactive healthcare management.
Key Responsibilities:
* Review and sign off on home blood pressure monitor reports.
* Conduct comprehensive Health Assessments and Heart Health Assessments.
* Develop and manage Chronic Disease Management (CDM) plans including but not limited to diabetes and cardiovascular conditions.
* Initiate and manage Warfarin Management Plans.
* Implement and monitor Diabetes Management Plans.
* Liaise with multidisciplinary teams including nurses, allied health professionals, and specialists.
* Ensure accurate documentation and compliance with relevant medical standards and protocols.
* Provide empathetic, patient-centered care within the home setting.
Qualifications & Requirements:
* MBBS or equivalent medical degree.
* Current registration with AHPRA as a General Practitioner.
* Experience in chronic disease management and preventative health.
* Strong communication and interpersonal skills.
* Ability to work independently and manage time effectively.
* Willingness to travel to patients' homes.
* Current driver's license and access to a vehicle.
* Experience with Warfarin and Diabetes management programs.
* Familiarity with electronic medical record systems.
