DV-RRH Case Manager - DV Rapid Rehousing Program
Under the supervision of the Domestic Violence Rapid Rehousing department, the Case Manager provides services to homeless and formerly homeless individuals and families fleeing domestic violence or human trafficking. The role involves using evidence-based practices such as Trauma-Informed Care, Motivational Interviewing, Harm Reduction, and Housing First.
The primary responsibilities include developing and implementing goals in the client's Individualized Service Plan (ISP), assisting clients in maintaining housing, and addressing issues like mental health, substance use, income, and interpersonal relationships. The position involves recertifying program eligibility, conducting home visits, and follow-ups to support housing stability and independence.
This is a non-exempt, union position earning $20.00 per hour, with working hours from Monday to Friday, 8:30 am to 5 pm.

Qualifications:
* Bachelor’s degree in social, behavioral, or human services
* At least one year of supervised experience in mental health, chemical dependency, or social services
* Valid Ohio driver’s license with less than six points and proof of automobile insurance
