Customer Care Specialist
Job Description:

* Develop Strong Relationships: You will work closely with families, clinical teams, and external providers to develop comprehensive care plans that cater to the individual needs of each customer.
* Coordinate Multidisciplinary Services: You will conduct onboarding assessments, coordinate services, and ensure customers receive safe and person-centred care at home or in a place of their choice.
* Support Care Transitions: You will assist customers during hospital discharge or changes in their health needs and follow up with offboarding where necessary.
Key Requirements:
* Experience: Community or aged care experience, including case management and coordinating multidisciplinary care in the home.
* Skills: Excellent communication skills, technical proficiency, and knowledge of funding streams and quality standards.
* Qualifications: Certificate IV or Diploma in Community or Aged Care desirable.
