End of Life Care (palliative care) Support Worker - Identified position

The End of Life Care (palliative care) Support Worker plays a vital role in delivering compassionate, culturally safe, and person-centred support to Aboriginal and Torres Strait Islander clients and their families during palliative and end-of-life care. Working under the guidance of the End of Life Care Project Officer, the Support Worker assists clients in accessing local palliative care services, attending appointments, and engaging with community resources. This role involves close collaboration with Bullinah clinical staff and external stakeholders to ensure holistic care that respects cultural practices and enhances quality of life. The Support Worker also contributes to community outreach, promotes death and grief literacy, maintains accurate documentation, transport of patients, and participates in ongoing training to support best practices in end-of-life care.
