What you will do:
* Provide high-quality and effective social work services, responding to the needs of patients and their families
* Complete assessments and support plans according to the Care Act 2014, adopting a strengths-based approach
* Work collaboratively with the hospital team to facilitate smooth patient discharges
* Manage risks and support patients to enhance their independence
* Undertake strengths-based Care Act assessments for patients and their informal carers
* Participate in regular supervision and training sessions to continuously improve your skills
* Work in line with our Supervision Policy to ensure you receive guidance and support
* Engage in professional development as expected by Social Work England
