Overview
Care Partner - Clinical - Home Care - Inner West role at HammondCare. The Care Partner (Clinical) works within the Mission and Mission in Action of HammondCare and plays a pivotal role in supporting older adults to live independently and well in their own homes. The role is responsible for client-centred care planning and clinical care management, and supporting the Client Care Operations Manager in achieving strategic objectives and growing the clients we care for at HammondCare.
This role requires a deep understanding of aged care services, a client-centred approach, and strong communication and coordination skills. You will work closely with clients, families, and multidisciplinary teams to develop and implement personalised care plans that align with each client’s unique needs, preferences, and goals.
Key Focus Areas
- Assess and understand individual client needs, including clinical needs and preferences, to develop tailored care plans that align with service expectations
- Coordinate and oversee client-centred care planning to ensure services are appropriate, responsive and person-focused
- Manage and monitor the delivery of in-home support services, ensuring compliance with Aged Care Quality Standards
- Build and maintain strong client relationships by providing ongoing support, communication and guidance throughout their care journey
- Collaborate with multidisciplinary teams to document, review and ensure compliance with care plans and all related activities
What do you need to apply?
- Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action
- University-level qualification in a relevant health-related discipline (e.g., Bachelor of Social Work, Physiotherapy or Nursing)
- Experience in aged care, home care, or community care, with a passion for client-focused service delivery
- Strong ability to develop, implement, and review personalised care plans
- Excellent communication and relationship-building skills with a commitment to advocacy and person-centred care
- Knowledge of Aged Care Quality Standards and compliance requirements
- Experience working collaboratively with healthcare professionals, carers, and external providers
- Strong problem-solving, organisational, and coordination skills
- Experience operating effectively in a multi-site environment
- A current driver’s licence and protected driving record; willingness to travel between HammondCare sites and services
- Excellent communication skills and ability to engage with clients from diverse backgrounds
- Strong administrative and computer skills
- Ability to work independently and as part of a team
- Empathy and patience
