About the role:
The role of Service Lead provides frontline leadership and coordination of a geographically based community care team. The role ensures the delivery of safe, person-centred and compliant services through effective coordination of care planning, rostering and incident response activities. The role is accountable for client outcomes, team performance and the promotion of local services. Services are delivered by Support Workers across a range of community care programs.
The primary objectives of the role include:
Lead a team of Care Partners and Support Workers;
Direct service promotion to support the growth and sustainability of the program and support the achievement of team KPI and budget targets;
Support workforce scheduling and delivery of high-quality, goal-directed client services;
Ensure compliance with Aged Care Quality Standards, HSQF and NDIS Practice Standards;
Coordinate intake, client onboarding and ongoing care planning processes; and
Oversee development and review of care plans, service agreements and budget utilisation.
What we are looking for:
Essential:
Minimum of five (5) years' experience in community care services, including care planning, service rostering and team supervision; and
Current Driver's license (QLD).
Desirable:
Qualifications in Community Services, Health, Aged Care, Disability or equivalent experience.
Core Competencies
Substantial leadership and coordination capability in the delivery of care services, with a proven ability to lead and manage service teams effectively;
High level of understanding of individualised funding models and associated compliance standards, ensuring services align with regulatory and funding requirements;
High level of skill and ability to deliver and monitor services that are responsive to, and aligned with, the individual needs and goals of clients;
High level of demonstrated capability to coach and guide teams to deliver safe, high-quality, person-centred care;
High level of attention to detail in overseeing day-to-day service logistics and rostering to ensure operational efficiency and service continuity;
Demonstrated ability to interpret and apply regulatory frameworks and ensure that care delivery consistently meets and exceeds applicable quality standards;
Demonstrated ability to undertake sales and promotional activities and successfully convert prospective client leads into signed service agreements;
Demonstrated commitment to continuous improvement, with a proactive approach to identifying and implementing opportunities to enhance service quality and achieve positive client outcomes; and
Demonstrated strong ethics, integrity, and adherence to confidentiality in handling sensitive client information, ensuring compliance with all relevant regulations, internal controls, and audit requirements.
