Dream Maker Community Services have an exciting opportunity to join our growing team as a Support Worker. This position will require you to have a valid Drivers License as it is mobile, in and around Greater Sydney.
We are looking for a passionate, empathetic, people orientated Support Worker with excellent communication skills (fluent English). To assist our Mental Health & Disability participants with their everyday living task, achieve their goals and engage in community-based activities.
**Your Role and Responsibilities, may include**:
- Assisting participants with their **NDIS focused goals** and daily activities
- Breaking down **every day living task** in order to build a consistent routine
- Prompting and or assisting with **personal hygiene**; showering, dressing, brushing teeth
- **Community focused activities**; planning and following through with social activities in and around participants local area, strengthening their social skills
- Assist with **in home care**, preparing meals, assisting with washing and various household task
- **Transport**, to and from medical appointments, activities, and outings
- Be able to create an innovative approach to best **interact** with participants individual needs
- Ensure to always be an **active listener** so that you can build on and strengthen your professional Support Worker role
- Some participants may require RN experience, manual handling, and industry specific health task such as peg feeding, bowel care, tracheostomy needs. (RN experience is not necessary for all participants)
- Must have clear **communication and writing skills**, capable of completing detailed **Shift Summaries** in a timely manner and communicate any **incidents** accordingly
**Essential Requirements**:
- Relevant support worker experience / training
- National Police Check
- Working with Children Check
- Valid Australian Driver's license and a reliable, insured vehicle
- NDIS Worker Check
- Covid - 19 Vaccine is Mandatory or would need an exemption letter.
- **Innovative thinker with a can-do attitude, that can adapt to various participants needs and actively use their common sense to identify and minimise risk.**
**Desirable Requirements**:
- Qualifications AIN, RN, Mental Health, Disability, Community Supports and/or Individual Support
- First Aid & CPR
- ABN
- Experience working with people who have a disability and challenging behaviours
- Adequate communication and verbal skills
- Reasonable writing skills for all mandatory reports
- 6 months or more experience in a relevant industry
- Flexible availabilities and willing to drive to various participants residencies
- A proven track record of commitment and reliability
