Recruiting for a full time Case Manager / Care Partner
We are currently looking for a full-time Case Manager/Care Partner to join our Sunshine Coast team Monday to Friday, 8.30am – 4.30pm. The Case Manager/Care Partner performs an integral role in assisting in individualised services being delivered to our customers in their homes, ensuring our participants can remain living their optimal lives, safely at home for as long as possible.

Duties and responsibilities may include:
1. Working with or under the guidance of medical practitioners and Allied Health professionals
2. Collaborating with clinical and experienced colleagues and a very supportive admin team
3. Ensuring care plans are monitored and reviewed on a regular and ongoing basis and in response to changes in participant needs
4. Ensuring services are delivered are in line with the person centred and consumer directed principles
5. Monitoring and evidencing or participant's wellbeing and documenting progress of participants
6. Working with participants to help them participate in the daily activities they want to do, including hobbies, their interests, to moving about their home easily
7. Ensuring that all services are provided in a caring and respectful way in accordance with Just Better Care's policy and procedures, legislative requirements and program specific guidelines
8. Assist in participant service schedules including service planning, confirmation and short notice placements or cancellations
9. Liaison with Aged Care Assessment Team (ACAT), My Aged Care (MAC) Portal, PRODA and other regulatory bodies
10. Assist participants to access other alternative services where the services are no longer meeting their needs
11. Undertake administrative duties including data entry, processing invoices, following up participant enquiries, ensuring documenting all interactions ensuring compliance etc.
12. Ensuring evidence is gathered and uploaded including quotes, confirmation of delivery of services etc.
13. Liaise with relevant community based agencies and service providers to ensure an integrated and cohesive approach to service delivery and to ensure standards are met in accordance with program objectives
14. Work with participants with their budgets and monitor participants' budgets in accordance with program guidelines.
15. Communication with key stakeholders, internally and externally.

* Identify and participate in personal professional development.
* Apply and uphold principles of equity and anti-discrimination in the workplace and adhere to organisational and legislative Health, Safety and Environment requirements
* Auditing of participant files regularly to ensure compliance
* Conducting skills assessments of Community Support Professionals who perform complex care services as required
* Demonstrating a commitment to best practice and evidence of continued professional development
* Participating in staff meetings and attending training opportunities when required
* Any other duties as required to meet the ongoing needs of the organisation
What you will need:
17. Strong administrative, computer, time management and organisational skills
18. Strong communication skills and outstanding Customer service
19. Keen eye for detail, impressive follow up and tracking/ auditing methodology
* Current CPR / First Aid Certificate
* Current and satisfactory Criminal Record Check
* Current and satisfactory Federal Police Check, NDIS Workers Screening Card & Blue Card
* Current Australian Driver's Licence
* Reliable, registered and insured vehicle (comprehensive insurance is preferred)
Desirable:
20. Previous experience in the community care or health/aged care sector
21. Previous experience working with home care Compliance
22. Understanding of Community Nursing guidelines or previous community nursing experience
23. Cert IV in Workplace Training & Assessment
* Cert III in Individual Support
