SAS Clinical Aged Care Assessment Officer & Delegate
Fixed Term Contract - 12 months
Responsibilities
* Undertake client assessments, reviews and development of support plans to assist clients in achieving their goals and living independently in their homes and community.
* Conduct assessments across all Northwestern Melbourne Local Government Areas (LGAs).
* Participate in clinical triage and case conferencing.
* Identify, monitor and manage risks and lead appropriate escalation.
Qualifications
* Tertiary qualification in a health‐related discipline directly related to health, aged care or a specialist area such as allied health, social work, or registered nursing.
* Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA) or another relevant professional association.
* Enrolled nurses with at least one year of aged care needs assessment experience are also considered.
Additional Requirements
* Successful candidates must undergo a National Police Check, International Police Check (if applicable) and hold a valid WWCC.
* Full driver licence and the ability to travel between sites, including all Northwestern Melbourne LGAs.
Employment Details
This is a 1‐year funded appointment with the possibility to extend.
