The Role
This position is responsible for leading the student transition program, coordinating activities such as parent information sessions, Masterclasses and Orientation programs for students aged 15 to 18 transitioning into the college.
The Transition Coordinator will provide individualised case coordination and brokerage services, offering guidance to students, families and staff and connecting them with relevant external providers based on their needs.
Responsibilities
* Develop and implement plans for young people to access suitable pathways to recreational, training and employment opportunities.
* Contribute to developing systems and intervention strategies that deliver responsive support services while maintaining strong networks with local youth, education, employment and counselling services.
* Maintain accurate records, analyse data and prepare reports to support individual plans.
Eligibility Requirements
* Hold an eligible teacher qualification and registration.
* Be an Australian or New Zealand citizen, permanent resident or have a valid visa with work rights.
* Apply for a WA Working with Children Check before starting; and
* Obtain a current Screening Clearance Number issued by the Department of Education's Screening Unit before starting.
