**About the role**
GH at Home (HITH) is an innovative program designed to reduce in-hospital length of stay and prevent unnecessary hospital admissions by delivering acute care to suitable patients in their homes. As the GH at Home Medical Officer, you will work under the direct supervision of the Director of General Medicine, providing high-quality clinical care to patients enrolled in the GH at Home service at Grampians Health.
**Key Responsibilities**:
Collaborate with clinicians in ED, wards, DPU, preadmissions, and inpatient services to facilitate the transfer of appropriate patients to GH at Home, minimising hospital admissions
. Establish and maintain effective working relationships with acute inpatient and ED clinical units to ensure seamless patient care
. Actively participate as a member of the GH at Home team, including nursing and administrative staff, to deliver the highest standard of care
. Contribute to continuous quality improvement within the GH at Home program
. Work closely with department heads to identify and develop priority areas for the GH at Home program
**Skills & Experience**:
MBBS or equivalent degree
. Full General AHPRA Registration
. Medical Officer HM14+
. Minimum two years' experience in an Australian hospital
. Relevant clinical experience and competence appropriate to the position
. Strong problem-solving, negotiation, and conflict resolution skills
. Ability to make informed decisions and effectively communicate them to relevant stakeholders
