We are seeking a dedicated and compassionate individual to join our team as a Support Practitioner. In this rewarding role, you will have the opportunity to make a positive difference in the lives of people with disabilities, supporting them to develop and maintain their independence.
Your key responsibilities will include:
* Providing personalized support to clients, assisting them with daily living tasks, personal care, and medication administration.
* Transporting clients in our wheelchair-accessible vehicles and providing community access.
* Monitoring client well-being and reporting progress to ensure optimal care.

To be successful in this role, you will need:
* Experience in disability support, care, or a related field.
* A current NSW Driver's License (P2 minimum).
